Six-Year-Old With Life-Threatening Grade IV Liver Injury Saved at SGPGI
Lucknow: A six-year-old boy with a life-threatening liver injury sustained in a road accident was successfully treated at the Apex Trauma Centre of Sanjay Gandhi Postgraduate Institute of Medical Sciences (SGPGI), Lucknow, through emergency surgery and multidisciplinary trauma care.
Krishna Kumar, son of Vikram and a resident of Nawade Parsoni village in Gopalganj district of Bihar, suffered serious injuries after being hit by an e-rickshaw on July 29, 2026. He was referred to the Apex Trauma Centre for specialised trauma management and admitted on August 1.
Investigations revealed a Grade IV liver injury, with nearly two litres of blood accumulated inside the abdomen (hemoperitoneum) and ongoing bleeding from the injured liver. Considering the severity of the injury and continued internal bleeding, an emergency surgery was performed on the same day. The primary objective was to control the bleeding and stabilise the severely injured liver.
The surgery was performed by the trauma surgery team, while the anaesthesia and critical care teams provided intensive perioperative support. Following surgery, the child required mechanical ventilation for around 48 hours. He was subsequently weaned off ventilator support as his condition improved.
The child remained under close observation for recurrent bleeding, infection, respiratory complications and other possible consequences of severe abdominal trauma. With coordinated surgical, anaesthesia and critical care management, his condition steadily improved.
Krishna was discharged from the Apex Trauma Centre on August 19, 2026, in satisfactory condition.
According to the treating team, Grade IV liver injuries can cause life-threatening blood loss, particularly in young children. Rapid assessment, timely referral, emergency control of bleeding, specialised trauma surgery, advanced anaesthesia support and postoperative critical care are crucial for successful outcomes in such cases.
